Individualised Community Support

The Individualised Community Support service provides 1:1 or 2:1 flexible support for people with learning disabilities who may have additional support needs, to access a full range of activities in their local community.

Individualised Support Plans
A person-centred Support Plan is developed with each service user to include a range of activities and outcomes designed to meet the individual’s personal preferences and requirements.

Below are some of the activities our service users have taken part in:
• Education and training
• Access to work
• Volunteering
• Exercise, sport and leisure (for good health and well-being)
• Gardening
• Learning for independence e.g:-
  ~ Travel
  ~ Cooking
  ~ Money management

Community Bases
Our service provides a base for service users to access if they wish or need to; here they may choose, for example, to have lunch, access a computer or chill out.
